  • Paraprofesionals - Braintree Open Roads

    DATE AVAILABLE: ASAP

    QUALIFICATIONS:
    • High School Diploma or equivalent.
    • Valid Driver’s License.
    • Basic knowledge of Microsoft Word and the Internet.
    • Successful completion of all state and agency mandated trainings.

    COMPENSATION: Competitive

    JOB DUTIES:
    • Provide support to individuals in a 1:1 or group format as defined in the ISP/DHSP.
    • Responsible for writing reports, including daily logs, utilizing professional, appropriate documentation skills.
    • Work in cooperation with Case Managers to develop member skills in the areas of: selfcare, self-preservation, expressive communication, self-direction, and activities of daily living.
    • Accurately record all data for established goals and behavior plans.
    • Implement all plans as outlined by OT, PT, Speech, Nursing and Behavioral objectives for individual members.
    • Serve as a liaison between RTR and the community representing a positive and proactive attitude at all times.
    • Serve as an appropriate role model for individuals under supervision establishing
    behavioral limits in a positive consistent manner and in accordance with behavioral
    plans.
    • Implement all behavior plan interventions as assigned. These behavioral interventions may include physical restraint.
    • Adhere to all program routines and schedules.
    • Provide transportation to community activities as needed.
    • Assist and support individuals as needed with physical direct care. May include:
    physical or mechanical transfer of individuals from wheelchairs to a van, chair, toilet etc.

  • Paraprofessional - Day Habilitation Program

    Provide support, evaluation and treatment to achieve OT, PT, speech, Nursing and Behavioral objectives.

    Provide DIRECT CARE supports in a 1:1 or group setting.

    Develop member skills such as self-care, self-preservation, expressive communication, and activities for daily living.

    Serve as an appropriate role model representing RTR in a professional and positive manner.

    Implement behavior plans including physical intervention.

    Assist with personal care.

    Attend community outings.

    Schedule: Full-time Monday-Friday 8:00am-4:00pm
    Part-time schedules may also be available.

    *Requires a minimum of one year of previous direct care employment experience at a community facility for individuals with developmental disabilities.

    Management experience in Human Service required. Related degree preferred.

  • Case Manager

    Provide direct care support at a residential setting. Develop skills for self-care, safety, communication, and activities of daily living. Complete general housekeeping, cooking, and household tasks. Provide assistance with personal care, medical appointments, community access, implement behavior plan strategies, etc. Develop goals and maintain documentation and communication with team members.

  • Employment Specialist

    Provide direct job support at an employment program and at community based sites.

    Oversee activities relative to vocational rehabilitation.
    Enhance social and workplace skills.
    Supervise the production and the quality of the work. Example: assembly, packaging, labeling, cleaning at a workshop style program.
    Package materials, skid the cartons of the completed work.
    Implement plans for vocational objectives, write job assessments, task analysis and daily payroll documentation.\
    Implement behavior plans including physical intervention.
    Assist with personal care as needed.
    Serve as an appropriate role model representing RTR in a professional and positive manner.

    Schedule: Monday-Friday 8:30a-4:00p
