- Employment Specialist
Provide direct job training support to individuals with disabilities working at an employment program and community based sites.
Oversee activities relative to vocational rehabilitation.
Enhance social and workplace skills.
Supervise the production and the quality of the work.
Write progress notes, production reports, payroll and task analysis.
Implement behavior plans including physical intervention.
Assist with personal care as needed.
Serve as an appropriate role model. - Case Specialists
Provide customized support, evaluation and treatment for individuals with developmental disabilities. Write monthly progress notes, individual support plans and objectives for annual reviews. Assist in the development of behavior plans and implement behavior plans as assigned. Either a Bachelor in Human Services, Counseling, Psychology or Medical Science or an Associates Degree plus 18 months of training in either: Human Services, Counseling, Psychology or Medical Science fields required. ANY SUITABLE COMBINATION OF EDUCATION, TRAINING AND/OR EXPERIENCE IS ACCEPTABLE so long as the applicant has at least the requisite Bachelor Degree or Associates Degree and 18 months of relevant training. - Registered Nurse - Day Habilitation Program
Hours Monday through Friday, 9 - 3
Provide general nursing care. Coordinate medical care with each recipients primary care provider. Administer medication, maintain documentation fo rthe individuals condition and progress notes, reaction to drugs, medical incidents, and treatments. Provide oversight to maintenance-therapy treatment. Respond to life saving situations. Monitor vital signs. Maintain awareness of comfort and safety needs. Participate in quality improvement activities. Maintain communication of relevant healthcare to professionals and family/guardian.
Requirements: Bachelor's degree and a minimum of one year working as an RN in a community health setting. Valid Registered Nurse Certification. Computer literacy, maintain valid drivers license.
- Licensed Practical Nurse
Provides general nursing care to individuals in Residential Services.
Qualifications:
Bachelor's degree and one to two years related experience and/or training.
Must hold valid Practical Nursing License.
Valid Driver's License.
Basic knowledge of Microsoft Word and the internet.
Must be fluent in English.
Ability to apply common sense understanding to carry out detailed written or oral instructions. - Lead Case/Case Manager/Direct Care
Assist the residents with daily activities such as scheduling and attendance of social activities, medical appointments, meetings, and other community events. General household tasks and assistance with their personal care. Develop and implement behavior plan intervention strategies which could include physical restraint. Good writing and communication skills needed to write reports, attend and participate in meetings and maintain positive interaction with team members. Maintain a valid driver's license. Shift schedules vary and may include hospital and rehab stays at off-site locations. - Program Director/Administrator - Day Habilitation Services
A minimum of five years of professional health care experience subsequent to a Bachelor's degree. This must include clinical experience and at least two years in a mental retardation program. One out of the two years must have been spent in an administrative role and one year of supervisory experience.
Oversee the development of individual service plans developed within the Day Habilitation program.
Advise management and Vice President in development and maintenance of habilitation process.
Provide training and education to staff relating to individuals needs.
Interpret habilitation functions in interdisciplinary planning and coordination of medical and therapeutic service programs.
Conduct regular meeting with staff.
Develop, implement, and modify as necessary, "mini" Exposure Control Plan to comply with OSHA's Bloodborne Pathogens Standard. Train staff accordingly.
Perform all personnel management functions in cooperation with Human Resources and in accordance with RTR's policies and procedures.
This job has supervisory responsibilities for all Paraprofessional and Case Management staff and for all staff in the absense of VP.
